Job Description: Advanced Practice Practitioner
Full-Time/Part-Time
Location: 841 Case St.
The Free Clinics seeks a compassionate and skilled Advanced Practice Practitioner (APP)
to join our dedicated team. This role can be filled by a Physician Assistant (PA), Nurse
Practitioner (NP), or Advanced Pharmacist Practitioner (APP). The APP will deliver high quality, patient-centered care to our patients, emphasizing accessibility, equity, and
holistic health outcomes.
Key Responsibilities

  • Provide comprehensive medical care, including assessment, diagnosis, treatment, and
    management of medical conditions in the medical clinic and bridges to health groups.
  • Collaborate with a Medical director and Physicians to develop and implement
    individualized care plans.
  • Conduct health education and preventive care services to promote wellness and
    empower patients.
  • Prescribe medications, therapies, and interventions following state licensure and
    organizational policies.
  • Maintain accurate and timely patient records using EMR.
  • Participate in quality improvement initiatives to enhance clinical outcomes and patient
    satisfaction.
  • Serve as a clinical resource for staff and patients, fostering a culture of collaboration and
    learning.
  • Engage in community outreach efforts to support the mission of providing equitable
    healthcare access.
    Qualifications
  • Current licensure or certification as a Physician Assistant, Nurse Practitioner, or
    Advanced Pharmacist Practitioner in North Carolina.
  • Master’s degree in a relevant healthcare field required; Doctorate preferred.
  • Minimum of 2 years of clinical experience (preferred).
  • Strong commitment to serving underserved and diverse populations.
  • Excellent interpersonal, communication, and organizational skills.
  • Ability to work effectively in a team-based environment.
  • Proficiency in Spanish is a plus.
